Creatinine Clearance Formula (Cockcroft-Gault Equation)
For Males:
CrCl (mL/min) =
( (140 – Age) × Weight (kg) ) ÷ ( 72 × Serum Creatinine (mg/dL) )
For Females:
CrCl (mL/min) =
( (140 – Age) × Weight (kg) ) ÷ ( 72 × Serum Creatinine (mg/dL) ) × 0.85
